Diabetes Team
The Community Diabetes Service in the Purbeck is a nurse led service that supports healthcare professionals in primary care, who are managing diabetic patients with the aim to reduce hospital admissions, and patients with diabetes who are managing their condition to the best of their ability, following the most up to date guidelines and technology.
The service is delivered through a combination of face to face clinics located at six GP surgeries across Purbeck, as well as telephone clinics, an advice line service, domiciliary visits and various education programmes for people living with diabetes, designed to best meet the needs of the individual. The Service also provides care to patients in nursing and residential care homes. Patients can be referred to our services by their healthcare professionals, through the Diabetes Purbeck referral system, or they may have direct contact with the service if previously under the community specialist nurse care
